Case Management 

Our staffs conduct community-based assessment and planning process by working with consumers, families, service providers, and other agencies to create an individualized plan based on a comprehensive assessment of needs for each person.

 

Case Management Services include the following:

  • Coordination of medical care with the primary care provider.

  • Maximum use of resources to improve quality of care.

  • Clinical Assessment

  • Case Monitoring and/or Clinical

Additional services include home care and facilitating provision of an emergency

response system, medical equipment, therapy, etc.
